2025 ACA Form 1095-C Distribution Notice

As part of the Affordable Care Act (ACA) requirements, East Lansing Public Schools prepares Form 1095-C for applicable employees.

The Paperwork Burden Reduction Act, signed into law on December 23, 2024, allows employers to no longer automatically distribute Forms 1095-B and 1095-C to covered individuals unless a copy is requested.

East Lansing Public Schools will continue to file all required ACA forms with the Internal Revenue Service (IRS) in accordance with federal deadlines.

Employees who would like to receive a copy of their Form 1095-C may submit a request using one of the following methods. Please include the employee’s full name, mailing address, email address, and phone number.

In accordance with federal regulations, requested forms will be provided within 30 days of receipt. Requests may be submitted at any time. The District’s ACA forms will be filed with the IRS by March 31, 2026.
